The following forms are required to be filled out by New Sea Cadets and League Cadets.

1. Click on each form title below and continue with the next several instructions.

2. Once the form displays, click on the first Blue Shaded (may be white) box and enter your information.  Then tab to each additional shaded box and enter your information.

3. Once you have entered all of your information, use your Browser Print Button to print the form.

4. Click the Browser Back Button and continue to fill out and print each additional form.

5. Bring the completed and printed forms to your Orientation or if you have already attended your orientation, bring the completed forms to the first drill weekend.
